It's a VERY expensive keyword, so the people paying for it need to know what they are doing right?

Check the headlines of those ads and you'll see something weird. Notice how almost no one actually uses the word "divorce attorney" in the headline of their ad?

That's weird, because it's the first thing someone is going to write when searching for that service.

And it's easy to set up as well. Just do some aikido with the keywords and ad groups and voila! You show up at the right time for the right word.
